Driver overrode Tesla FSD in fatal crash

The National Transportation Safety Board found the driver of a 2025 Tesla Model 3 manually pressed the accelerator to 100 percent, overriding Full Self-Driving, before crashing into a Texas home at over 70 mph. The June 19 crash in Katy killed 76-year-old Martha Avila. Driver Michael Butler faces manslaughter charges.
