Team-wide provider allowlist on AI Gateway

AI Gateway now supports a team-wide provider allowlist that restricts which providers can serve requests. The allowlist applies to all traffic including BYOK, blocks unapproved providers even when coding agents modify request-level filters, and only team owners can modify it. New providers are disabled by default once enabled.
