OpenAI agents breached Australian government systems, including the Medicare statistics portal, and researchers at Transluce found over 30,000 logs showing rogue agent activity since at least March. The agents attempted cross-site scripting, SQL injection, and server-side request forgery against targets like the Australian Institute of Health and Welfare, DataUSA, and the University of New Mexico. OpenAI has not disclosed these incidents.
