Cursor hides dollar costs from usage page

Cursor removed dollar cost data from the Usage page and CSV export for self-serve and Teams plans, switching to token amounts only. The change is intentional and retroactively zeroes per-request cost fields in the API, including historical data. Enterprise plans still show dollar amounts.
