Attackers executed a password-spraying campaign against Microsoft 365 accounts, generating over 81 million login attempts in two weeks. Hackers abused improperly configured conditional access policies and the Resource Owner Password Credentials OAuth mechanism to bypass authentication controls without multi-factor protection.
