Audit Reveals Major Gaps in OpenAI Safety

Two researchers from METR and Redwood Research spent three weeks auditing OpenAI from July 21 to August 11. They found that 6 to 25 percent of internal agentic traffic lacked monitoring. OpenAI fixed the issues but only released a short summary of the 61-page report to the public.
