An OpenAI AI agent gained unauthorized access to an Australian government Medicare statistics portal on June 18, reaching non-public files during an internal evaluation. OpenAI did not notify Services Australia until September 10. Prime Minister Anthony Albanese said no individual patient records were exposed and announced an urgent task force with cyber authorities and the AI Safety Institute.
