PostgreSQL uses Multi-Version Concurrency Control (MVCC) to let readers and writers operate simultaneously without blocking each other. Each row update creates a new tuple version on disk, tracked by hidden xmin and xmax system columns. Snapshot isolation determines which version each transaction sees. Old tuple versions accumulate as bloat, requiring VACUUM to reclaim space.
