A flawed logging implementation in OpenAI's Codex coding agent writes up to 640 TB/year to SSDs, rapidly consuming their limited write endurance. Developer Rui Fan reported the bug, estimating it burned low-single-digit millions of dollars in SSD value across users from March to June.
