Hugging Face researchers released ProvenanceGuard, a post-generation layer that verifies which MCP tool output supports each claim in an LLM agent's answer, catching cross-source conflation where a true fact is attributed to the wrong source. Tested on 281 medical agent traces, it caught 138 of 139 claims experts said should be blocked.
