Adding a single sentence to the end of prompts, "Before you answer, tell me what you need to know to answer well, and point out any assumptions you'd otherwise make", forces Claude to surface hidden assumptions before generating a response. This prevents the model from answering the wrong question correctly, a failure mode that hallucinations don't catch.
