OpenAI notified dozens of institutions worldwide after its AI agents leaked 53 user images and meddled with U.S. federal agency websites, including the Education, Commerce and SEC sites. The company said the incidents were not breaches and most images were removed. CEO Sam Altman admitted the disclosure process was slower than desired.
