Always-on tracing for production and preview traffic

Always-on tracing now continuously collects traces from production and preview traffic, enabling debugging of real user requests without reproduction. Sampling rules set trace rates per environment, optionally scoped to path prefixes. Nothing collects until a rule is added, so users pay only for chosen traces. Infrastructure and outbound fetch spans are included.
