Extend eve agents with installable extensions

Eve now supports installable extensions that package tools, connections, skills, instructions, and hooks as reusable mounts. Extensions are not copied into agents, enabling upgrades via a package manager. A single command scaffolds a new extension, creating the package, installing dependencies, and initializing Git. Consuming an extension requires one file to mount it under an agent.
