Better Embeddings Won't Fix Missing Provenance in RAG

A production RAG system answered a parental leave question with a 2021 policy, revised twice since, for three weeks. The embedding model scored the obsolete chunk 0.008 higher, and the table stored no source, date, or version. The fix requires adding provenance metadata to the retrieval layer.
