NTSB: Driver overrode Tesla FSD in fatal crash

Michael Butler manually pressed the accelerator to 100 percent, disabling Tesla's Full Self-Driving (Supervised) system before fatally striking Martha Avila in her Texas home. The NTSB found the vehicle's speed exceeded 70 mph, the brake was never applied, and Butler was charged with manslaughter.
